Standard Registration Form. Use this registration form for all players except U9 and U6 (Kick and Chase). Send the form, along with payment, to the registrar. Note that players returning for the Fall season will be sent a packet containing this form and a stamped envelope addressed to the team's coach. If you receive this packet, you should use that form and send payment to your coach in the provided envelope.

U9 Registration Form. Use this registration form for players registering for U9 teams. U9 players will be in 3rd grade during the Fall soccer season, so with a few exceptions they will be under 9 years old (U9 = Under 9) at the start of the season. Again, players returning for the Fall season will be sent a packet as described in the description of the Standard Registration Form above.

U6 (Kick and Chase) Registration Form. Use this registration form for kids who will be kindergarteners during the Fall season for which you are registering. Send the filled out form along with payment to the registrar.

New Players - Registration Checklist. If your child has never played soccer with an MTSC team before, read through this document to make sure you get your child registered properly.

Returning Players - Registration Checklist. If your child has played with an MTSC before, look here to make sure you have gotten them properly registered.

Field Request Form. Coaches should use this form to communicate their practice field needs to our Fields Coordinator. Print a copy, fill it out, and deliver it to the fields coordinator.

Equipment Form. Coaches should use this form to order any equipment they may need for their team. Print a copy, fill it out, and deliver it to the equipment representative (see Contacts).

Merchandise order form. Due to the changing availability of some of our merchandise, we are currently making changes to our order form. Check back soon to buy cool MTSC gear.

Coaches' Pledge. All MTSC coaches need to read and sign this form, and have it filed with the club while they are coaching.
